Preliminary Loss Reports (PLR): 
This link will lead you to the location to sign up to receive PLRs.  PLRs are descriptions of how Soldiers have lost their lives.  It is a sobering look at how we lose our brother/sisters in Arms and provides lessons learned for safety briefings or leader development sessions.
